*New cheerleader Registration with Full Uniform, Warmups (pants and jacket), Shoes & Accessories Fee total: $600.00 - this includes tax and shipping
*Returning cheerleader who does NOT need a uniform- i.e., shell, turtle neck, skirt, brief, duffle bag Fee total will be $300 (if you need any of the above items your cost will be $300 plus the cost of the items you need)
**Non- refundable deposit for everyone required today- $125.00
IMPORTANT INFORMATION:
All uniforms take 8 to 10 weeks to process. Depending on when your total fees are paid in full, this is when your uniform will be order.
After submitting this Registration Form and Deposit Payment, the Cheer Director will contact you to schedule a fitting time.
For registration after June 13, 2025 - the family will be given a size chart to send in measurements for the uniform. We will no longer have a sizing kit after June 13, 2025. Please note all uniforms are custom made and non refundable.

Medical Physical Form Document
All participants must have a certified Medical Sports Physical completed before Helmet & Equipment Distribution Day. Helmet/Equipment will not be issued until the Medical Sports Physical is submitted. Players will need to submit official documentation proof of a Medical Sports Physical signed by a certified medical professional and must include a medical certification stamp or the medical professional's license number with office address and phone number and must be dated on or after January 1st of the current year. The document must specifically indicate that a medical or sports physical has been performed. *Many local walk-in clinics can provide a Medical Sports Physicals.

Cheer Showcase: Late October 2025 (TBA)
Playoffs End: Late November 2025
Practice Days: Tues, Wed, Thurs ~ 5:30p to 7:30p @ Milpitas Sports Complex Football Field
**First month of practice is every weekday, M-F until school begins in mid August**
Game Days: Saturday
**Game could be on a Sunday by exception if the away game team cannot secure a field on Saturday
